She will explain the multiple choices that are available and lay out as many options as possible. She will provide you with the forms needed to fill out, steps required for different types of care, and guidance along the way.

We will begin over the phone discussing all your needs, wishes, and concerns. If a follow-up in person assessment is deemed necessary, we will come to you. This is a time to gather information about current living situations, activities of daily living, medical conditions, lifestyle preferences, budget, location, timeline, and any questions or concerns you may have.

We will work together to develop a personalized care plan that outlines the level of care, type of facility or services needed, and any specific preferences or requirements.

We will do all of the leg work to research the homes and communities, providing you with the information needed to make informed decisions.

We will go with you to tour homes and communities, helping with questions and guiding you through the process.

We will have a consultation to provide the best possible options for your needs and wish list. This can include personal guided tours if you are looking to move in the near future.
